US: 'Novel' flu strain found in Weber County (Utah)
OGDEN -- A Weber County resident has tested positive for an unusual strain of influenza that has hospitalized three people in the U.S. and is not included in this year's vaccination. The virus has been identified in isolated cases across the nation, including West Virginia, Maine, Iowa, Pennsylvania and Indiana.The Weber County resident was not hospitalized and is recovering at home.
The virus, A/H3N2v, was confirmed by the Centers for Disease Control and Prevention and the Utah Department of Health. The strain is genetically similar to those which circulate in pigs, said Weber-Morgan Health Department director, Gary House.
"Fortunately, this individual is getting better," House said of the Weber County resident. "We know there have been 12 other cases within the past year in the United States that have also fully recovered."
House said the health department is working closely with the CDC, the state health department and the Utah Department of Agriculture and Food to determine how the local resident was exposed to the new strain. House said as a precaution,those who have been in contact with this person have been advised to watch for fever, cough and other flu-like symptoms. So far, this seems to be an isolated case and is the only one confirmed in the state, according to the health department.
Health officials called the virus "novel," meaning any case of human infection with influenza A that is different from the currently circulating and predicted strain, said Amy Carter, Weber-Morgan Health Department communicable disease nurse. She said it is common practice to monitor diseases that are capable of transmitting from animals to humans. "We want to stay as ahead of the game as possible so we can know how to best be prepared," Carter said. "This doesn't mean the strain will be included in next year's vaccination. That is determined by what strains we are dealing with this year and what is circulating worldwide." Continued:

OGDEN -- A Weber County resident has tested positive for an unusual strain of influenza that has hospitalized three people in the U.S. and is not included in this year's vaccination. The virus has been identified in isolated cases across the nation, including West Virginia, Maine, Iowa, Pennsylvania and Indiana.The Weber County resident was not hospitalized and is recovering at home.
The virus, A/H3N2v, was confirmed by the Centers for Disease Control and Prevention and the Utah Department of Health. The strain is genetically similar to those which circulate in pigs, said Weber-Morgan Health Department director, Gary House.
"Fortunately, this individual is getting better," House said of the Weber County resident. "We know there have been 12 other cases within the past year in the United States that have also fully recovered."
House said the health department is working closely with the CDC, the state health department and the Utah Department of Agriculture and Food to determine how the local resident was exposed to the new strain. House said as a precaution,those who have been in contact with this person have been advised to watch for fever, cough and other flu-like symptoms. So far, this seems to be an isolated case and is the only one confirmed in the state, according to the health department.
Health officials called the virus "novel," meaning any case of human infection with influenza A that is different from the currently circulating and predicted strain, said Amy Carter, Weber-Morgan Health Department communicable disease nurse. She said it is common practice to monitor diseases that are capable of transmitting from animals to humans. "We want to stay as ahead of the game as possible so we can know how to best be prepared," Carter said. "This doesn't mean the strain will be included in next year's vaccination. That is determined by what strains we are dealing with this year and what is circulating worldwide." Continued:

CIDRAP: WHO reports progress on H1N1 pandemic mortality estimates
The World Health Organization (WHO) is working on a preliminary global mortality estimate for the 2009 H1N1 pandemic, and it recently posted an update on the work of a technical expert group that is in the early stages of gauging the impact. Two teams are working with the WHO to produce independent estimates with different methods, one from the US Centers for Disease Control and Prevention and one from the Netherlands Institute for Health Services Research.

The WHO has said the number of confirmed 2009 H1N1 pandemic deaths is a gross underestimate (Snip) better estimates of the cases and deaths are needed to better understand the impact of the pandemic and how it measures up historically.
"While the data for a definitive estimate of mortality will likely not exist for several years, and will never exist for some parts of the world, a preliminary estimate would be useful in the short term, and would serve to give a more realistic understanding of the true impact of the event than currently available reported mortality figures," (Snip)
Initial work presented at the October meeting was based on limited data from industrialized countries. As the next step, more countries, including some middle and low income countries, will provide data for the project this year, which should help the two groups further refine their estimates by the end of the year.

SENSENBRENNER - Preventing release of lethal science
When researchers used federal funding to genetically mutate the lethal H5N1 bird flu virus to make it capable of respiratory transmission between ferrets, the U.S. government was caught flat-footed on how to proceed with this potentially dangerous research. This week, a Senate panel is investigating biological security in the wake of the H5N1 research, with good reason.
Initially, the National Science Advisory Board for Biosecurity (NSABB) recommended against publication of the research. The board initially believed that any benefits from publishing the research were outweighed by the risks that the information could be used as a recipe for terrorists seeking to create biological weapons. But when the National Institutes of Health asked the board to reconvene three weeks later, the NSABB recommended in favor of publication.
Then in a leaked letter, Michael T. Osterholm, a member of NSABB and director of the Center for Infectious Disease Research and Policy at the University of Minnesota, wrote that the decision to reverse its position on the H5N1 studies was based on a biased presentation of the evidence and was "a lot less about a robust science and policy-based risk-benefit analysis and more about how to get us out of this difficult situation."
The controversy surrounding the H5N1 study and the criticisms in Mr. Osterholm's letter confirm my suspicions that the U.S. government is woefully unprepared for dealing with dual use research of concern - research that, while conducted for a legitimate scientific purpose, could be dangerous if misused. Egypt's Real Crisis: The Dual Epidemics Quietly Ravaging Public Health

A combination of avian flu and foot and mouth disease risk destroying the protein supply, eroding public trust, and further destabilizing the Arab world's most populous country.
Lost in the recent political jockeying and protest violence leading up to Egypt's May 23 presidential elections is the unfolding public health disaster there. Avian flu and foot and mouth disease are running rampant, killing people and livestock as well as inflating the price of food. It's a serious health and economic issue, but it has potentially much larger implications for Egypt, and this little-discussed crisis is beginning to resemble those that occur in failed states.
The Egyptian state, which was not particularly well-prepared for public emergencies even before the February 2011 revolution brought it into near-chaos, has little capacity to cope with the outbreaks threatening not only Egypt, but also Sudan, Israel, the West Bank, Gaza Strip, and Jordan. Egypt's public health infrastructure barely functions. The sort of social service that groups like the Muslim Brotherhood have provided over many years, it falls far short of what is needed to combat the current crisis. Cairo does not have the money to throw at the problem, having burned through more than half of its foreign currency reserves in the 15 months since Mubarak's fall.
Ground zero for Egypt's public health emergency is Libya, where last year, in the midst of civil war, foot and mouth disease swept through the country, killing more than 10 percent of its sheep and cattle. Smugglers subsequently brought infected sheep across the Libyan border, setting off a foot and mouth disease (FMD) wildfire that Egyptian officials have been unable to slow.
Within four weeks, FMD killed thousands of cattle, buffalo, sheep, goats, camels, and other livestock across Egypt. In March, the United Nations Food and Agriculture Organization declared a "catastrophe," warning that the epidemic in Egypt was threatening human food supplies for all of the Middle East and North Africa. The particular viral strain responsible for the epidemic, officially called SAT2, is a new one, against which standard FMD vaccines are useless. SAT2 was first found in Sudan in 1977, when it was also thought to have been eliminated. It appears to have resurfaced there in 2010, spread to Libya, then Egypt, and now the Palestinian Territory. Consequently, public and veterinary health officials in Israel, Saudi Arabia, Jordan, and elsewhere around the region are on high alert.
For Egypt, which is reeling from the economic repercussions of last year's revolt against the Mubarak regime and ongoing political instability, the FMD epidemic is a serious blow. The loss of thousands of cattle, buffalo, and camels has resulted in a significant spike in the price of meat from Egypt's remaining livestock, which leaves a relatively poor population with only one unaffected source of protein: chickens. Yet Egypt is also in the sixth year of avian influenza H5N1 epidemic. Despite vaccination and control efforts, the deadly H5N1 virus, which swept into Egypt from Asia, persists. Given the popularity of home-raised chickens in the country, where many households, rural and urban, possess flocks, controlling the infection would be a daunting undertaking for any government. Millions of the animals are estimated to have either died from infection or been killed off by veterinary authorities, according to the UN's Food and Agriculture Organization, in failed control efforts. The World Health Organization now ranks Egypt second in the world, after Indonesia, in human cases of the avian flu, which thus far hasreportedly killed 60 Egyptians and infected about 100 more. Recently, a strain of the virus has spread to Egyptian ducks; this new strain carries mutations that are thought to play a role in enabling transmission between mammals. This is a particularly worrisome development because some Egyptians have caught the flu from their animals, but not yet passed it onto other people. Once the virus begins to spread between humans, an epidemic becomes far, far more likely.
There are only two well-established methods for controlling these viral animal diseases: through mass vaccination of more than 90% of all vulnerable animals, or via quarantine and culling measures that identify and slaughter all FMD-infected herds and H5N1-contaminated flocks. In either case, governance is the key. These are big, difficult undertakings, and they require public trust in the state. Poor farmers are far less likely to cooperate with such a program, which asks them to risk, their livliehood, with faith that government will provide adequate compensation for culled animals.
In Egypt, where the military's ability to exert its authority has diminished over time, the transitional civilian cabinet is ineffective, and a variety of political forces jockey for advantage as the presidential election draws near, the government looks increasingly unable to deal with the FMD crisis, and people are furious about it. When more than 7,000 cattle died in March, protesters took the streets to demonstrate al Gharbiyya governorate. Unable to get help from the state or compensation for dead animals, impoverished famers dumped animal carcasses into water systems and canals, and on the doorsteps of local officials. By late April, the animal death toll topped 20,000, as the infection spread to the Red Sea and South Sinai governorates as well as the Palestinian territories. When local veterinary experts tried to impose quarantines on infected herds, camel drivers threatened to block Egypt's main highways, insisting on the right to use their beasts regardless of contamination concerns.
In early April, the Egyptian government announced it would buy $1.8 million worth of a vaccine manufactured to treat the specific strain of FMD threatening Egypt's livestock. The first stocks arrived at the end of April. Though vaccinations have begun, it is unclear when they will become widely available. Even if a mass vaccination of Egypt's hoofed livestock could be accomplished, despite the uncertainty of Egypt's politics and limited capacities of its sprawling bureaucracy, it will -- like all vaccines -- take two weeks before the benefits of immunization take effect. Under the best-case scenario, control of FMD would not be realized until June or July. As for bird flu, it's doubtful that Egypt could stop the viral spread of the disease, portending more human cases and deaths.
Observers have spent a lot of time and effort trying to understand Egypt's post-Mubarak trajectory, but this path will be much more than just the twists and turns of Egyptian politics. Public health is a critical factor for the country's transition and its future. Even if presidential elections are smooth, if the military hands power to civilians with no problems, and if Egyptians get down to the hard work of writing a constitution, uncontrolled foot and mouth disease and avian flu -- along with their attendant economic and human costs -- could risk everything. If the international community wants to help guide the Arab world's most populous country through its post-Mubarak crisis, addressing the country's twin epidemics would be a particularly high-impact way to do it. Absent a global response, Egypt will likely experience continued political instability and violence, and potentially widespread malnutrition that would surely affect Libya, Sudan, Gaza, Israel, the West Bank, Jordan, and beyond. Endless debates about the Muslim Brotherhood's commitment to democratic change or the military's intentions are interesting, but not as important as a coordinated international effort to pull Egypt back from the public health brink.

The CDC has released another series of H1N1pdm09 sequences at GISAID. The newly released sequences confirm and extend the clonal expansion of oseltamivir (Tamiflu) resistance due to H274Y. Earlier sequences had H274Y in the five most recent sequences from Texas, as well as the four most recent sequences from Mexico, including cases from Mexico City. The newly released sequences include four Texas isolates collected in March (Snip) and all four had H274Y and were closely related to each, other as well as the earlier cases in Texas and Mexico.
In addition, two recent isolates from California (Snip) and Illinois (Snip) also had H274Y and were also closely related to the sequences from Texas and Mexico, signaling a broader clonal expansion.
The CDC's week 19 Fluview notes that most of these isolates from cases not receiving anti-viral treatment, as well as two cases where family members were receiving treatment. These observations support transmission of H274Y.

The recently released H1N1pdm09 sequences confirm and extend clonal expansion of H274T tamiflu (oseltamivir) resistance in Mexico, Texas, California, and Illinois. All of these cases are closely related to each other, signaling clonal expansion. Moreover, the week 19 FluView confirms that 13 of the 16 cases had no exposure to Tamiflu, while two cases were family members of patients being treated with Tamiflu, confirming clustered transmission of H274Y.
(Snip) two of the cases (60F and 19M) also had D225G (Snip), raising concerns that D225G and H274Y are transmitting. The genetic background of these cases matches a dominant 2012 H1N1pdm09 genotype that is widespread in the United States, indicating the sub-clade is well adapted for spread and raising concerns that H274Y and D225G are circulating as mixtures, and the true level is markedly higher than the recently reported cases.

Med residents lacked PPE knowledge, worked while ill in pandemic, study finds
A study of doctors in training during the height of the 2009 H1N1 pandemic found gaps in personal protective equipment (PPE) knowledge and guidance and that some worked when they were sick.
The study (Snip) was prompted by a cluster of influenza-like illnesses in internal medicine residents in June 2009, which led to a National Institute for Occupational Safety and Health (NIOSH) investigation. At the time, flu activity in Utah, the state where the medical school was located, was widespread. The investigators visited the medical school and four affiliated hospitals in September 2009 and conducted an electronic survey of residents about their PPE practices. Researchers also explored records of doctors' contacts with sick patients, peers, community members, and family members and reviewed PPE recommendations that were in place at the time.
Of 210 residents, 88 (42%) completed the survey. Thirteen reported flulike illness during the time period, and among 8 who sought medical care, 5 had lab-confirmed influenza A. (Snip) 85% said they had been exposed to the pandemic virus in a variety of settings, and 10 said they had worked while ill.
A cluster of 8 illnesses occurred after an internal medicine residency program dinner. Many of those who worked while sick cited professional obligations to patients and coworkers. From 13% to 88% said they knew PPE recommendations (Snip) three hospitals deviated from interim federal PPE guidance, partly because of a shortage of N95 respirators.

The Ministry of Health of El Salvador confirmed today that swine flu cases rose to 50 in recent days, but denied that there is an outbreak of this disease,which caused no fatalities so far this year.

He denied that it is an epidemic, but "are isolated cases of the disease."
Most cases seen this year has had "fewer complications" because the damages were "less severe than in previous years" due to the vaccination process which has already been three years, he said.
The Director of Health Surveillance, Ministry of Health of El Salvador, Julio Armero, said recently that the flu is "circulating" in the country since 2009 and in 2010 and 2011 there was a "low road" of the virus.
"But at the regional level (Central) is seeing the H1N1 this year will be circulating with greater intensity," said Armero.
The H1N1 virus in Guatemala had caused at least five dead and more than 55 patients until 12 March.
Between June and August is when most circulating influenza viruses common and A, so earlier this month Health launched a vaccination campaign at national level to counter them.
The Directorate General of Health Surveillance an increase of the notification in health regions such as Concepcion, Guaira, Caaguazú and Caazapá. They urge people to go to consultation at the first signs of the disease, especially to vulnerable groups. In epidemiological week 20, total visits for influenza-like illness amounts to 101,556 notifications, as highlighted by Dr. Andrea Ojeda, in a press conference.
The professional said that there was an increase of 27.2% compared with the previous week, which, however, is considered normal for this time of year. "There is an increase, particularly in health areas like Concepcion, Guaira, Caaguazú and Caazapá, so we can say that we are entering a channel epidemic, although it is expected, compared with figures from previous years," he said.
As for the movement of viruses, noted that the ratio indicates a prevalence of 33% of adenovirus, followed by pandemic influenza A H1N1, with 26%. "The most affected age group is the covering 15 to 59 years," he said, at the time noted that the proportion of deaths by IRAS does not exceed 5%. (Snip)

New bird flu virus killing US baby seals

WASHINGTON (AFP) – A new kind of bird flu has been causing deadly pneumonia in baby seals off the northeastern US coast and could pose a risk to humans, according to US research released Tuesday.
The new strain has been named avian H3N8, and is blamed for the deaths of 162 seals along the US coastlines last year, said the study in mBio, a journal of the American Society for Microbiology.
Most of the dead seals were younger than six months of age.
While there have been no known human cases to date, scientists at Columbia University in New York urged caution, given the history of bird flu and its ability to evolve into forms that can infect people, like H5N1.
“Our findings reinforce the importance of wildlife surveillance in predicting and preventing pandemics,” said W. Ian Lipkin, professor of epidemiology at the Columbia University Mailman School of Public Health.
“HIV/AIDS, SARS, West Nile, Nipah and influenza are all examples of emerging infectious diseases that originated in animals,” Lipkin added.
“Any outbreak of disease in domestic animals or wildlife, while an immediate threat to wildlife conservation, must also be considered potentially hazardous to humans.”
Scientists sequenced the full genome of the new strain and found it originated from a bird flu virus that had been circulating in North American waterfowl since 2002.
Over time, the virus gained the ability to infect mammals by latching on to receptors in their respiratory tracts.
Wildlife experts first grew alarmed in September 2011 when an increasing number of seals from the coasts of Maine to Massachusetts began developing pneumonia and skin lesions.
A total of 162 dead or dying seals were recovered over the next three months, the researchers said.
Early research on the mutations in the strain “suggest enhanced virulence and transmission in mammals,” though further study is needed, the authors said.
The bird flu that most people know about, H5N1, remains relatively rare but has killed about half of the people it has infected since the first outbreak in Hong Kong in 1997.
The World Health Organization has tallied 606 human cases of bird flu since 2003 and 357 deaths, according to a June report.
The so-called swine flu, or H1N1, broke out in Mexico in 2009. The H1N1 virus spread into a global pandemic that claimed the lives of 17,000 people.
